PLEASE READ CAREFULLY BEFORE SIGNING By participating in the 9th Annual Heads Up For Our Youth Back to School Rally, you acknowledge and agree to the following terms: 1. Injury Liability Waiver I, the undersigned, hereby release and hold harmless Heads Up For Our Youth, its directors, officers, employees, volunteers, and any third-party affiliates from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the participant while engaged in activities related to this event. I assume all risks associated with the activities, whether foreseen or unforeseen, and voluntarily choose to participate in the event. 2. Image Release I hereby grant permission to Heads Up For Our Youth and its third-party affiliates to capture and use photographs, video recordings, or other visual media of myself and/or my child(ren) during the event for purposes including, but not limited to, promotional materials, advertisements, documentaries, and any commercial use. I understand that these images may be used in print, digital, or broadcast media, and I waive any rights to royalties or other compensation arising from their use. 3. Parental Consent (For participants under the age of 18) As the parent or legal guardian of the minor(s) participating in this event, I consent to the terms outlined in this agreement on their behalf. I acknowledge that I have the legal authority to provide such consent. 4. Agreement to Terms By checking the box below, I confirm that I have read and understood this Image Release and Injury Liability Waiver Agreement, and I agree to its terms in their entirety.
